répondre: It comes with what you see in the photo. My throne had the hole on the bottom for mounting the back rest. It all depends on what throne you have. -

The attachment fits with the Gibraltar 9608OS and 9608HB models and can also fit to the softy round throne (9608-SFT) if the bottom has a threaded connection point. The attachment does not come with mounting brackets and isn't compatible with a Gibraltar 6608 throne.
